Anchor House Coordinated Entry and Assessment

Coordinated Entry works with the City of Trenton Coordinated Entry System to assess and prioritize youth and young adults who are experiencing homeless.

Service beyond housing...

Anchor House’s Youth Coordinated Entry Assessment System Team (CEAS) works with youth ages 18-24 years of age who are experiencing homelessness. CEAS works closely with all community partners serving our population to streamline and coordinate the navigation of youth through housing services. This team is integrated into The City of Trenton Coordinated Entry and Assessment System under the Continuum of Care.

The CEAS Team assists clients with:

  • Housing assessments
  • Crisis intervention
  • Case management
  • Referrals to social services
  • Referrals to housing options
  • Referrals for inpatient and outpatient services for substance abuse & mental health.
  • Linkage to the Anchor Link Street Outreach team for any gateway services needed. (food, clothing, showers, etc.)
